I've received an unknown FormSwift charge on my card. Can anyone help with this?
I have a charge for $37.00 on my debit card, and I don't know how to get help with that. I did not agree to have an account.

You need to speak to Form Swift direct: https://formswift.com/contact
Note. If you DID NOT give your card details to the site then you need to speak to your bank because somebody has cloned your info.
